Patient Financial Responsibility

Thank you for choosing Liahona Treatment Center for your treatment services.  We are committed to providing quality and affordable mental health care. To better address your questions regarding patient and insurance responsibility for services rendered, please review the payment policies below.  It is important for the guardians of all patients to understand these policies along with their insurance benefits. Insurance Liahona Treatment Center participates with all insurance plans that provide Out-of-Network Mental Health Benefits.  Even if you are insured by a plan that provides Out-of-Network Mental Health Benefits, payment for treatment is required each month until your mental health coverage can be verified.  Knowing your insurance benefits is your responsibility. Please contact your insurance company with any questions you may have regarding your coverage. If you are not insured by a plan that provides Out-of-Network Mental Health Benefits, or if you do not have any health insurance, payment is expected as contained in your Admission Contract. Co-payments and Deductibles All co-payments and deductibles must be paid at the time of treatment unless prior payment arrangements have been made.  This arrangement is part of your contract with your insurance company. Failure on the part of Liahona Treatment Center to collect co-payments and deductibles from patients can be considered fraud. Please pay your co-payment upon admittance into Liahona Treatment Center if prior payment arrangements have not been made.  Depending on your insurance coverage, Liahona Treatment Center may request pre-payment of anticipated deductibles, co-insurance and co-pays for upcoming treatment services.  Prepayment Requirements Prepayment of anticipated charges or a commitment to make scheduled monthly payments may be required in the cases of no insurance coverage, especially for residential treatment care or partial hospital care.  Please be aware that some, or perhaps all, of the services you receive may not be covered or not considered reasonable or medically necessary by your insurance provider. In these situations, you must pay for these services in full. Self-Payment Requirements An uninsured or “cash-pay” discount may be applied to all self-pay accounts at an agreed upon discount rate for patients that meet certain qualifications.  If you are unable to make full upfront payment for the services to be provided, a mutually agreed upon monthly payment arrangement may be made between the responsible party and Liahona Treatment Center. Proof of Insurance Liahona Treatment Center must obtain a copy of your photo ID and current valid insurance to provide proof of insurance. If you do not provide us with correct insurance information in a timely manner, you may be responsible for the balance of a claim. Insurance Claims Submission Liahona Treatment Center and/or its third-party insurance processor, will submit your claims and assist you in any way we reasonably can to help get your insurance claims paid. Your insurance provider may need you to supply certain information directly to them. It is your responsibility to comply with their request. Please be aware that the balance of your claim is your responsibility whether or not your insurance provider pays the claim. Your insurance benefit is a contract between you and your insurance company; Liahona Treatment Center does not participate in that contract. Overnight Housing Liahona Treatment Center offers housing (room and board) separate from the treatment setting.  Patients admitted in outpatient care at Liahona Treatment Center have the option of keeping the patient within Liahona’s housing facilities and paying for the housing services directly.  This is highly recommended by Liahona as routine attendance to the treatment center is usually hindered by a lack of transportation and the patient’s normal home environment may be undermining to the patient’s recovery process.  Typically, most insurance providers do not cover housing (room and board) when the patient is being treated with outpatient care. If housing is elected by the responsible party, Liahona Treatment Center will submit a monthly bill to the responsible party for the agreed upon housing obligations.Coverage Changes If your insurance coverage or insurance provider changes, please notify Liahona Treatment Center immediately so we can make the appropriate changes to help you receive your maximum benefits. If your insurance company does not pay your claim within a reasonable time, the balance will be billed to you. Nonpayment If your account is over 60 days past due, you will receive a letter from Liahona Treatment Center. If a balance remains unpaid after 90 days past due, Liahona Treatment Center may refer your account to a collection agency.  Interest in the amount of Eight Percent, (8.0%), per annum, will be charged on all open account balances that are more than 60 calendar days past due.
